What a mini gastric bypass involves

The mini gastric bypass — one-anastomosis gastric bypass, OAGB — creates a long, narrow stomach pouch and joins it directly to a loop of small intestine 150–200 cm down, bypassing the rest. There is one join instead of the two of a Roux-en-Y bypass, so the operation is shorter (about an hour), has fewer leak points and a lower rate of internal hernia, with weight loss and diabetes remission that are at least equivalent. It is now one of the most commonly performed bariatric operations in Europe and endorsed by IFSO.

The trade-off is a slightly higher risk of bile reflux and of nutritional deficiency than a sleeve, which is why lifelong vitamin supplementation and annual blood tests are part of the plan, not an option.

Who is eligible

BMI 35 and above, or 30 and above with type 2 diabetes or another obesity-related condition; age 18–65; previous serious attempts at weight loss. Patients with severe pre-existing reflux are usually steered toward a Roux-en-Y bypass instead. Pre-operative blood tests, ultrasound and an anaesthesia review are done on arrival.

Desk work at two weeks, sport at six. The dietitian follows you remotely at one, three, six and twelve months; you take a daily multivitamin, B12, iron and calcium for life.

Frequently Asked Questions

Mini bypass or sleeve?
The sleeve is simpler with fewer nutritional issues; the mini bypass produces more weight loss and better diabetes remission and does not worsen reflux the way a sleeve can. The bariatric team recommends based on BMI, reflux and diabetes.
Mini bypass or classic Roux-en-Y?
Weight loss is comparable. The mini bypass is shorter with fewer complications; the Roux-en-Y is preferred if you already have significant reflux.
Will I need supplements for life?
