Yes, a Brazilian butt lift (BBL) and a tummy tuck can be done in the same trip, and often in the same operation, when the patient is healthy, at a stable weight and a non-smoker, and when the surgeon keeps the total operating time within limits. The combination is not risk-free. Insurance data on US board-certified plastic surgeons (CosmetAssure) show that adding procedures to a tummy tuck raises the rate of serious complications, mainly blood clots. This guide explains what changes when you combine the two, what a partner surgeon in Istanbul will check first, and how the recovery and the trip home are planned.
Why the combination is popular
The two operations share one step: liposuction. A tummy tuck removes loose skin and tightens the abdominal muscles, while a BBL takes fat by liposuction from the flanks, back or thighs and grafts it into the buttocks. Doing both under one anaesthetic means one recovery period, one set of time off work and one journey. On Estetica's live price list, a BBL in Istanbul starts from €4,050 and a tummy tuck from €3,690, each as a package price covering surgeon, hospital and hotel nights at Deniz; flights are never included. A combined operation is quoted per patient after the surgeon has reviewed your photos and medical form.

What the evidence says about risk
The largest data set comes from CosmetAssure, an insurer for board-certified US plastic surgeons. Winocour and colleagues (Plastic and Reconstructive Surgery, 2015) analysed 25,478 tummy tucks and found that abdominoplasty already carries a higher rate of major complications than other cosmetic operations, and that combining it with other procedures increased that rate further. Venous thromboembolism, a clot in a leg vein that can travel to the lungs, was among the most serious events.
A second CosmetAssure study by Winocour and colleagues (Aesthetic Surgery Journal, 2017) looked at 129,007 cosmetic patients and found that combined procedures were an independent risk factor for clots. This does not mean the combination should be avoided; it means it needs a patient who fits the criteria and a team that runs a clot-prevention protocol. Our guide to combining several procedures in one trip covers the general rules that apply to any pairing.
Fat embolism and the BBL safety rule
The BBL has its own specific risk: fat entering the large veins under the gluteal muscle. After fatal cases, the Multi-Society Task Force for Safety in Gluteal Fat Grafting, backed by ASAPS, ASPS and ISAPS, issued a rule: fat must be placed only in the subcutaneous layer above the muscle, never into or under it. Combining the BBL with a tummy tuck does not change this rule, but it adds operating time, which is why the surgeon plans the order of the steps carefully. Ask your partner surgeon on the video call how they confirm the cannula stays above the muscle.
Operating time and how clot risk is scored
The American Society of Plastic Surgeons Venous Thromboembolism Task Force (Murphy and colleagues, Plastic and Reconstructive Surgery, 2012) recommends scoring every patient with the Caprini risk assessment before surgery. In that model any operation longer than 45 minutes counts as major surgery, and points are added for age, body-mass index, hormonal contraception and a personal or family history of clots. A combined BBL and tummy tuck is always major surgery, so the score decides whether you receive compression stockings, calf-pump devices and blood-thinning injections.
If your score or the planned operating time is too high, the surgeon may split the plan into two stages. That is a safety decision, not an upsell, and it is the main reason a combined quote can change after the surgeon reviews your file.
Who is a good candidate
Partner surgeons look for the same things before agreeing to combine: a stable weight for several months, a body-mass index within the surgeon's limit, no nicotine for the weeks the surgeon specifies, no personal history of clots, and normal blood tests on arrival. Enough donor fat is also needed for the BBL; in very slim patients the tummy-tuck area cannot supply it, because surgeons limit liposuction over the abdominal flap to protect its blood supply. If you plan to lose more weight or have more children, the tummy tuck is usually postponed.
The positioning conflict during recovery
The two operations ask opposite things of your body. After a BBL you avoid sitting or lying directly on the buttocks so the grafted fat is not compressed. After a tummy tuck you walk and rest slightly bent at the waist to protect the abdominal stitches. Combined patients sleep on their side or front with pillows, use a special cushion that keeps weight off the buttocks when sitting, and walk short distances several times a day from the first day to keep blood moving in the legs. A compression garment covers both areas.
Planning the trip and flying home
A combined operation means a longer stay than either procedure alone, because the surgeon must check the abdominal wound and the buttocks before clearing you to fly. The two separate guides on flying after a BBL and flying after a tummy tuck explain the checks; in a combined case the later of the two dates applies. From Italy the flight is short: Milano Malpensa (MXP), Milano Bergamo (BGY) and Roma Fiumicino (FCO) to Istanbul Airport (IST) take about two and a half to three hours, but you will still be sitting on a cushion, wearing compression and walking the aisle.
Clot risk does not end when you land. Our article on blood clots after surgery and flying lists the warning signs, such as one swollen, painful calf or sudden shortness of breath, that need emergency care at home. Bring a companion if you can: the first days after a combined operation are harder to manage alone.
Booking steps with Estetica
Start with a free video call with our assistant and send photos plus the medical form. A partner surgeon reviews them and decides whether a one-stage combination fits you. You then receive an offer; the €30 surgeon video call is the first paid step and is credited if you book. A €500 deposit holds your date, refundable if you cancel at least 14 days before surgery and before the flight is booked, otherwise kept as a 12-month credit.
Frequently Asked Questions
Can a BBL and a tummy tuck be done in one operation?
Yes, for a suitable patient. The partner surgeon combines them under one general anaesthetic when your weight is stable, you do not smoke, your clot-risk score is acceptable and the total operating time stays within their limit. If any of these fails, the surgeon may split the plan into two stages separated by a recovery period.
Is it safer to do a BBL and a tummy tuck separately?
Each operation alone is shorter, and CosmetAssure data published by Winocour and colleagues show that adding procedures to a tummy tuck raises the major-complication rate. Two stages mean two anaesthetics, two recoveries and two trips. The surgeon weighs your Caprini clot-risk score and the expected operating time to decide which option is safer for you.
How do I sleep after a combined BBL and tummy tuck?
You avoid lying flat on your back so the buttocks are not compressed, and you keep slightly bent at the waist to protect the tummy-tuck stitches. Most patients sleep on their side or front supported by pillows, and use a BBL cushion when they have to sit. Your partner surgeon gives you the exact routine and how long to follow it.
Will there be enough fat for a BBL if I also have a tummy tuck?
Usually the fat comes from the flanks, back and thighs rather than the abdomen, because surgeons limit liposuction over the tummy-tuck flap to protect its blood supply. Very slim patients may not have enough donor fat for the volume they want, and the surgeon will tell you so after reviewing your photos.
How much does a BBL and tummy tuck cost in Istanbul?
Estetica quotes the two operations separately on its live price list: a BBL from €4,050 and a tummy tuck from €3,690, each covering surgeon, hospital and hotel nights at Deniz, with flights paid by you. For a one-stage combination the partner surgeon first reviews your photos and medical form, then a single personal quote is issued.
