Compression Garment After BBL: Week-by-Week Guide for Patients Flying Home From Istanbul

After a Brazilian butt lift (BBL), the compression garment stays on 24/7 for the first month, coming off only to shower, and is then worn at least 12 hours a day, usually overnight, during month two. That schedule comes from Dr. Daniel Funderburk, writing for the American Society of Plastic Surgeons (ASPS). Your partner surgeon in Istanbul sets the final protocol, and ASPS itself notes there are no firm guidelines for how long each garment is worn. This guide walks through the stages week by week, so you know what to pack, what happens before you fly home and when the garment can finally come off.

What the garment does after a BBL

A BBL combines liposuction, usually of the abdomen, flanks and back, with fat transfer to the buttocks. The garment works on the liposuction areas, not on the newly grafted buttocks. ASPS lists its benefits as reduced swelling and inflammation, better circulation, less bruising and fewer hematomas, and a lower risk of complications such as blood clots. Dr. Kim, quoted in the same ASPS article, adds that patients who skip the garment have more swelling, more discomfort and need more lymphatic massage sessions.

BBL garments (a Colombian-style faja or a surgical bodysuit) are usually designed to leave the buttocks uncompressed or lightly supported. Pressing hard on the grafted fat is exactly what the recovery is designed to avoid, which is also why the sitting rules described in our week-by-week guide to sitting and sleeping after a BBL run alongside the garment schedule.

Stage 1: the first two weeks, in Istanbul and the first days home

The first garment, called stage 1, goes on in the operating room or right after surgery. It is firmer than later garments. Dr. Kim, writing for ASPS about compression after body surgery in general, describes stage 1 as worn 24/7 for one to two weeks. Dr. Funderburk advises patients to stay mostly at home for the first four to five days after surgery, and Dr. Williams notes that most need some pain medication during that period.

In practice the first part of this stage falls within your stay in Istanbul; the rest you finish at home. The partner surgeon checks the garment fit, the incisions and the drains (if any) at the follow-up visit before you are cleared to fly. How many nights to plan is covered in detail in how long to stay in Istanbul after a BBL. Bring or buy a second garment of the same size so one can be washed while you wear the other; the 24/7 rule allows only short breaks for showering.

What to check every day in stage 1

Look for creases or folds that dig into the skin, numbness or colour change in the legs, and a garment that slides down or rolls at the waist. Each of these is a reason to contact the partner surgeon through your Estetica assistant the same day, not after you land. Do not adjust zips or hooks tighter on your own initiative.

Stage 2: weeks 2 to 6

Once swelling begins to settle, surgeons typically switch you to a stage 2 garment, which is softer and more comfortable. Dr. Kim, in the same ASPS article, describes stage 2 as worn from about week two to week six. Dr. Funderburk still advises 24/7 wear, except in the shower, until the end of the first month. Swelling, not the calendar, decides the switch, so do not change garments before the surgeon tells you to.

This is also the stage in which most patients go back to work. Dr. Funderburk reports that patients return to work between days seven and 10, using a BBL pillow if they sit at a desk. For the first six weeks he limits sitting to roughly a 10-minute window at a time, which is hardest to follow on the flight home.

The flight home in a compression garment

From Istanbul (IST), the flight to Milan Malpensa (MXP), Milan Bergamo (BGY) or Rome Fiumicino (FCO) takes about three hours. Wear the garment on the plane, book an aisle seat so you can stand and walk every short while, and bring the BBL pillow so your weight sits on the backs of the thighs rather than on the buttocks. Our guide on flying after a BBL and how long to wait explains when the partner surgeon usually clears patients to fly and what to do on board.

A body-shaping faja alone is not a clot-prevention plan for the flight: it is not the same as graduated compression stockings for the legs, and only the surgeon can say whether you need stockings or blood thinners. Ask before you leave Istanbul, not at the airport.

Month 2: at least 12 hours a day

In the second month Dr. Funderburk recommends wearing the garment at least 12 hours a day, typically overnight. Another ASPS article describes this phase as night-time wear for several weeks after the surgeon approves it.

Keep follow-up photos going to your assistant on WhatsApp during this month. Send a front, side and back photo standing in good light, with and without the garment, at the points your surgeon asks for. The partner surgeon reviews them remotely and tells you when you can stop.

Months 3 to 6: when the result settles

According to surgeons quoted by ASPS, the risk of losing grafted fat subsides around month three, and between months three and six the shape refines and the curve becomes more visible. The same source puts average fat survival at about 60%, meaning the body reabsorbs roughly 40% of the transferred fat. That is why surgeons judge the final result at six months, not when you take the garment off.

Some surgeons ask for part-time wear for several months; ASPS quotes Dr. Hadeed recommending at least six weeks around the clock followed by part-time wear if needed. If your surgeon's schedule differs from the ones quoted here, follow your surgeon.

What a BBL through Estetica includes

A BBL in Istanbul through Estetica starts from €4,050. The package covers the partner surgeon's fee and hotel nights at Deniz; flights are never included. Your written quote states exactly which garments are supplied, so check it before you buy extras at home. The first step is a free video call with your assistant; after the offer, a €30 video call with the surgeon is the first paid step, and that €30 is credited if you book.

Quick timeline

Days 0 to 14: stage 1 garment, 24/7 except showering, mostly at the hotel for the first four to five days (ASPS). Weeks 2 to 6: stage 2 garment, still 24/7 until the end of month one; sitting limited to short windows for six weeks (ASPS). Month 2: at least 12 hours a day, usually overnight (ASPS). Months 3 to 6: fat-loss risk subsides around month three and the shape refines; average fat survival is about 60% (ASPS).

Frequently Asked Questions

Can I take the BBL compression garment off to sleep?

Not in the first month. ASPS guidance from Dr. Funderburk is to wear it 24/7 for the first month, removing it only to shower. In month two the pattern reverses: at least 12 hours a day, usually overnight, so sleep is when you are most likely to keep wearing it. Your partner surgeon in Istanbul confirms the exact switch date at follow-up.

How many compression garments should I bring to Istanbul for a BBL?

Plan on two of the same size, so one can be washed and dried while you wear the other. Stage 1 is worn 24/7 except showering, so a single garment leaves you without one while it dries. Your written Estetica quote lists which garments are supplied, so check it before buying extras at home.

Is a faja the same as compression stockings for the flight?

No. A faja or surgical bodysuit compresses the liposuction areas of the torso and thighs. Graduated compression stockings work on the lower legs to support blood flow. Whether you need stockings or a blood thinner for the roughly three-hour flight from Istanbul to Milan or Rome is a medical decision for the partner surgeon, so ask before you leave.

What happens if I stop wearing the garment too early after a BBL?

Dr. Kim, writing for ASPS, reports that patients who skip the garment have more swelling and discomfort and need more lymphatic massage sessions. The garment also helps limit bruising and hematomas in the liposuction areas. Follow the schedule your surgeon sets.

Should the BBL garment press on my buttocks?

BBL garments are usually designed to leave the buttocks uncompressed or only lightly supported, because strong pressure on newly grafted fat is what recovery tries to avoid. The compression is meant for the abdomen, flanks, back and thighs where fat was removed. If your garment flattens the buttocks or digs in, send a photo to your assistant so the partner surgeon can check the fit.
