That is a gap of €950 against Milan's entry price and €2,110 against the national average — real money, but far from the saving that medical-tourism advertising implies. What separates the two numbers is not the surgery. It is how much of the trip each one already contains.

Both figures leave out flights. Beyond that they are built differently: the Istanbul price arrives with five hotel nights, airport transfers and a written revision clause attached; the Milan price arrives with consultations and a day-surgery admission, and sends the pre-operative tests to a separate invoice. Price the items you would actually buy and the two quotes move toward each other.

What the €4,050 in Istanbul already covers

The package is built around surgery by a board-certified partner surgeon at a JCI-accredited hospital in Istanbul, with five package nights at the Deniz hotel, VIP airport transfers both ways, a patient coordinator who speaks your language, post-operative medication and a recovery kit, a follow-up programme running to twelve months with checks at three, six and twelve, and a twelve-month revision clause. Estetica Istanbul is an agency: the surgeons and the hospital are partners, and the surgical decisions are theirs.

What it does not cover is worth writing down before you compare anything. Flights are never included. Nights beyond the five cost €70 each at the Deniz, or €110 a night in an Antwell suite if you upgrade. Optional lymphatic drainage runs €100 a session, or €250 for three, and only when the surgeon advises it. Guidaestetica.it, which publishes figures collected from Italian specialists, reports a national gluteoplastica average of €6,160, with quotes in Milan spanning €1,500 to €9,500 and a technique-specific band of €2,000 to €5,000 for lipofilling or Brazilian butt lift. The same page notes that follow-up visits, at €50 to €100 each, are usually billed separately. A follow-up schedule bought visit by visit is a line item Istanbul buyers never see.

The low end of the Italian market is genuinely low. Clinica Pallaoro in Padova lists gluteal lipofilling at €2,000 to €3,000 and states that the figure covers the surgeon, the anaesthetist, all facility costs, dressings and post-operative checks. That undercuts Istanbul outright, with no flight and no hotel attached. It also buys a smaller operation: a modest-volume lipofilling is not the lipo-360-plus-transfer most people picture when they search for a BBL, which is the trap in reading any single number without the scope behind it. The hotel nights are the part people miscount

A Milan patient sleeps at home. An Istanbul patient sleeps in a hotel, and the only question is who paid for it. Five nights are inside the €4,050. If the surgeon asks you to stay longer, each additional night is €70, so three extra nights put the package at €4,260 and a week of extra nights at €4,540. Those are the numbers to compare with Milan's €5,000-plus-tests, not the bare headline.

Then add the flight, which neither price includes. Read the limit as well as the promise — travel and accommodation for a revision trip are still yours, and a second fat transfer is not something to assume. The first call is free and is with a patient assistant, not a surgeon. After the written offer, a video consultation with the surgeon costs €30, and that €30 is credited against the package if you book. Booking takes a €500 deposit. Cancel at least fourteen days before the date and before your flight has been booked, and the deposit is refunded; cancel later than that, or after the flight is booked, and it converts to a credit valid for twelve months. The practical consequence is an order of operations: deposit first, flight second.
