Frequently Asked Questions

What is the difference between DHI and FUE?
DHI uses a Choi implanter pen to extract and implant follicles in one step, while FUE creates recipient channels first and then places grafts separately. DHI allows denser packing and is ideal for hairline refinement. FUE is better suited for larger sessions over 3,000 grafts. The clinic team recommends the best technique for your case.
Is DHI more expensive than FUE?
DHI is slightly more expensive due to the specialised implanter pens and the higher precision required. At Estetica Istanbul, DHI starts from EUR 2,650 all-inclusive. The price difference is modest compared to the potential benefits for density and hairline work.
How many grafts can DHI handle in one session?
DHI is optimal for sessions up to 3,500 grafts. For larger sessions, FUE or a combination approach may be recommended. The clinic team will assess your needs during consultation and recommend the best approach.
Does DHI have a faster recovery than FUE?
Recovery timelines are similar. DHI may cause slightly less crusting in the recipient area since no pre-made channels are needed. Most patients return to normal activities within 7 to 10 days with either technique.
Who is the best candidate for DHI?
DHI is ideal for patients who need dense packing in a specific area (such as the hairline or temples), patients who want to avoid shaving the recipient area, and those seeking maximum naturalness in the frontal zone. The clinic team will advise during consultation.

DHI hair transplant in Istanbul — maximum density, no compromise

DHI — Direct Hair Implantation — is the most refined evolution of hair transplant technique. Using the Choi Pen (a specialised implanter pen), the technician team extracts and implants follicles in a single movement, eliminating the separate channel-opening step. The result is higher density, faster healing and greater precision in hairline design. DHI is our premium technique for patients who want the most natural result possible.

What changes between DHI and FUE

Both techniques extract individual follicular units. The difference is in the implantation phase. In classic FUE, the technician team first creates recipient channels with a blade, then inserts grafts into those channels. In DHI, the Choi Pen loads the follicle and implants it directly into the scalp in a single pass. This means:

  • No separate channel-opening stage — less total time with the scalp "open".
  • Follicles spend less time outside the body, which improves survival rate.
  • Angle, depth and direction of every graft are controlled simultaneously.
  • Higher implantation density is possible in a smaller area.
  • Less bleeding and faster scab shedding post-operatively.

DHI is particularly strong for the frontal zone, for temple reconstruction, and for patients who want to add density between existing native follicles without shaving everything off.

Unshaven DHI — why this is the preferred choice for professionals

One of the main reasons anglosphere patients choose DHI is the unshaven variant. Only the posterior donor area is shaved, and it is covered by the longer hair above; or we work with long hair in the donor area too. You return to work on Monday and none of your colleagues notice you had a procedure the week before. For anyone with a public-facing role, a senior position, or simply a preference for their medical choices to stay private, the unshaven option is the single most concrete selling point of DHI.

Who DHI is suitable for

DHI is the right option for patients who need high-density implantation in a specific area like the hairline or crown, want to add density without shaving existing hair (unshaven DHI is available), are after the most natural hairline possible, are early in their hair loss (Norwood 2–3) and want preventive thickening, or are women with diffuse thinning requiring implantation between existing hairs.

The DHI procedure, step by step

The DHI pathway follows the same clinical standard as every procedure we coordinate:

  • Free online consultation with photo analysis and personalised graft plan.
  • Pre-operative bloods at the partner JCI-accredited hospital.
  • Hairline design session with the clinic team.
  • Local anaesthesia with sedation options for comfort.
  • Extraction of individual follicles with micro-punch technique.
  • Direct implantation via Choi Pen (typically 2–6 pens used per session).
  • Post-operative PRP session to accelerate healing and graft survival.

A typical DHI session for 2,000–3,500 grafts takes six to ten hours. For more extensive cases, we may recommend a combined FUE + DHI approach, where FUE covers the crown and DHI handles the hairline for optimal result.

Results and recovery

DHI recovery is slightly faster than classic FUE thanks to less trauma to the recipient area. Most patients see initial redness subside within five to seven days. Transplanted hairs fall out between weeks two and four (shock loss — normal and expected), new regrowth emerges from month three. Maximum density is reached between twelve and eighteen months.

The precise angle control that DHI permits translates into an exceptionally natural result, especially at the frontal hairline. This is the reason many patients, even in combined procedures, specifically request DHI for the frontal zone.

The all-inclusive package

The DHI package includes everything you need:

  • DHI procedure with Choi Pen technology and medical oversight.
  • Hotel, three nights at the Deniz Hotel (extensible).
  • Private VIP airport transfers.
  • Pre-operative bloods and medical check.
  • Post-operative PRP treatment.
  • Medications, post-operative kit and specialist shampoo.
  • 18-month WhatsApp follow-up programme in English.
  • Procedure certificate with graft documentation.

Why not the low-cost DHI clinic

Many Istanbul and Balkan clinics sell "DHI" at €1,200 — but in reality they perform classic FUE and call the implantation phase DHI because it uses a generic pen rather than original Choi instruments. Real DHI requires original Choi pens (sterile, single-use or sterilised to protocol), a team trained specifically in the technique, and constant supervision. When the price drops below certain levels, one of those elements has gone missing.

What you pay for with us is verifiable: original Choi pens unsealed in front of you, a team trained specifically in DHI, a dedicated partner hair clinic with a JCI-accredited hospital on standby, 18-month English-language follow-up.

English-speaking team — from first message to 18-month follow-up

Our team replies in English on WhatsApp, phone and email. From the first quote to the twelve-month check, you always speak to someone who understands your language. Questions about regrowth, shock loss, when to return to the barber come up often in the first months — we want you to have immediate English answers when you need them.

Book your free consultation

Not sure whether DHI or FUE is right for you? That's exactly what the free consultation is for. Send us your photos via WhatsApp or the form on this page. The partner hair clinic's medical team assesses your pattern of loss, your donor density, and your goals, and recommends the technique that will deliver the best result in your specific case. Transparent quote, honest opinion, no obligation.

Ask what DHI changes in your individual treatment plan

A DHI proposal should explain more than the use of an implanter pen. Ask why this approach is being suggested for the areas you want treated and what alternatives were considered. Ask how donor assessment, hairline design, recipient areas, and implantation are organised.

Do not treat DHI as a guarantee of greater density, faster growth, or a higher survival percentage. Those are clinical claims requiring appropriate evidence and individual explanation. Ask what the team can reasonably discuss before examination and what can only be decided at the clinic.

If an advertisement says extraction and implantation happen without any intermediate handling, ask the clinician to describe the actual workflow. Your consent should relate to the procedure being offered, not a simplified comparison that turns a device into a promise of a better result.

No-shave expectations need precise questions

Tell the team how you normally wear your hair and what you mean by “no shave.” Do you want the recipient hair retained, the donor area concealed, or no visible trimming anywhere? Those are different requests. Ask which parts would be trimmed in your case and to what extent.

Request an explanation using a diagram or photograph so you can plan work and social commitments realistically. Ask whether redness, dressings, or other visible signs could still be apparent even if surrounding hair is retained. Retaining hair should not be marketed as a promise that nobody will notice treatment.

If discreet recovery is important, discuss your calendar before you pay. Explain uniforms, headwear, public-facing work, and any upcoming event. Ask what remains uncertain rather than building travel around an assumed invisible recovery.

Make the recipient-area plan explicit

Where existing hair remains, ask how the team assesses the area and explains the intended placement. Where an area is bare, ask what coverage the proposed donor supply could support. The same technique name can be used for different treatment aims; your plan should identify yours.

Ask to see appropriate cases similar to your starting pattern rather than only the most dramatic result. Check the interval between treatment and each photograph, and ask whether other treatment was used. Examples can support a discussion but do not predict your outcome.

For general information to discuss with your treating clinician, see American Academy of Dermatology's transplant information. It is general guidance, not evidence that DHI outperforms another implantation method or that a particular partner package will achieve a pictured result.

Who treats you and how medical questions are handled

Estetica Istanbul coordinates treatment with a partner hair clinic. Extraction and implantation are performed by its technician team, with a hospital on standby. Ask which clinician is responsible for assessment, anaesthesia decisions, and complications.

A technique or device does not establish professional credentials. Ask who is involved at each stage and how changes to the plan are discussed. Do not infer that the hair clinic has a particular accreditation from the relationship with a standby hospital.

Keep medical questions separate from itinerary questions. You should know who supplies the clinical instructions and who arranges accommodation, transfers, and administrative follow-up.

Compare the DHI price with the actual alternative

As of October 2026, DHI starts from €2,650 all-inclusive, flights excluded, with three package nights. The published FUE starting price is €2,350, making the headline difference €300. That difference is not proof of a clinical advantage. Ask what changes in your specific proposed treatment and which inclusions are the same.

Request confirmation of tests, medication, aftercare supplies, hotel, transfers, and follow-up. Flights, extra nights, and upgrades remain separate; additional Deniz nights are €70 per night and Antwell is €40 per night above Deniz. Ask how a changed plan after examination affects the quote.

Where the hair guarantee applies, it is an 18-month certificate with the condition that control photos are sent on days 1, 2, and 3, day 10, and months 1, 3, 6, and 12. Ask for the complete terms. The certificate does not justify an assumed survival percentage or remedy.

Frequently asked questions

Is DHI automatically better for me?

No automatic ranking should replace assessment. Ask why it is recommended for your goals.

Can I book a no-shave procedure from photos alone?

Ask what can be provisionally assessed and what requires examination. Confirm the actual trimming plan before proceeding.

Where do I ask for a proposal?

Use the DHI assessment page, stating your hair-loss concern, desired areas, and any shaving restrictions you want discussed.
