If you have enough spare fat on your abdomen, flanks or back, a Brazilian butt lift is usually the first option a surgeon discusses, and in Istanbul it is also the cheaper one: a BBL package starts at €4,050 and a butt implant package at €6,720, both with flights excluded. If you are very lean, the choice largely makes itself, because a BBL can only move fat you already have. That single anatomical fact matters more than the €2,670 price difference. What follows sets out how partner surgeons decide between the two, what each costs in Istanbul and in Italy, and the trade-offs in risk and recovery.
The deciding question: how much fat can you spare?
A BBL is two operations in one session: liposuction to harvest fat, then injection of the purified fat into the buttocks. The volume a surgeon can add is capped by what liposuction can safely remove, and part of the transferred fat does not survive, so the final shape settles over the following months. Patients with a slim build and little stored fat often cannot supply enough for a visible change. They are the main group for whom surgeons recommend silicone gluteal implants, which add a fixed, predictable volume regardless of body fat.

A BBL also reshapes the donor areas, which is why many patients prefer it: the waist and lower back get slimmer while the buttocks get fuller. An implant changes the buttock only. If you also want a slimmer waist, implants would typically be paired with liposuction, which adds to the price and the operating time. The partner team quotes the BBL package in Istanbul with the liposuction already included.
What each costs in Istanbul
The BBL package starts at €4,050 and covers the partner surgeon, a one-night hospital stay, hotel nights at the Deniz hotel and airport transfers. The butt implant package starts at €6,720 and covers the surgeon, the implants themselves, a two-night hospital stay, hotel nights and the same transfers. Extra hotel nights at the Deniz cost €70 each; upgrading to the Antwell costs €110 per night. A €500 deposit holds the date: it is refunded if you cancel at least 14 days before surgery and before your flight is booked, otherwise it becomes a 12-month credit.
The implant package costs more for a structural reason. A solid silicone gluteal implant is a manufactured device priced in euros or dollars wherever it is bought, so no local labour discount applies to it, and gluteal implant surgery is low-volume work in every market. Our country-by-country butt implant prices show that €6,720 is €2,037 below the UK average but €1,220 above Germany's entry price.
Istanbul vs Milano and Roma: what Italy charges for each
In Milan, Clinica Ireos publishes gluteoplastica from €5,000, with a working range of €5,000 to €10,000 depending on technique, and lists on-site pre-operative tests as an extra cost. The price aggregator Guidaestetica.it puts the Italian national average for gluteoplastica at €6,160. Against the €4,050 Istanbul BBL package, that is a gap of €950 to the Milan entry price and €2,110 to the national average, before you add the flight to Istanbul, roughly three hours from Milano Malpensa, Bergamo or Roma Fiumicino. Our breakdown of what a Milan BBL quote includes shows which Italian line items are billed separately.
For implants the picture reverses. Santagostino, the Milan-based clinic network, states that buttock implant surgery starts at a minimum of €5,000, and the Catania plastic surgeon Angelo Sapuppo publishes €6,500 to €8,500 for gluteoplasty with implants. The €6,720 Istanbul package is €1,720 above the Milan floor and inside the Catania range, so an Italian patient choosing implants is buying the package contents, not a discount.
Risk: what the evidence says about each
Neither operation is risk-free. A systematic review presented at the 2014 annual meeting of the American Society of Plastic Surgeons pooled 2,235 implant patients and 3,565 fat-grafting patients. Implants had an overall complication rate of 21.4%, led by wound dehiscence at 10.3%, seroma at 4.9%, infection at 1.9% and sciatic nerve paresthesia at 1.1%. Fat grafting had an overall rate of 9.9%, led by seroma at 3.5%, undercorrection at 2.2% and infection at 1.9%.
The most serious BBL risk is rarer but more dangerous: fat entering the large veins of the buttock and travelling to the lungs. The 2018 Gluteal Fat Grafting Safety Advisory, issued by a multi-society task force that includes the ASPS and ISAPS, tells surgeons to inject fat only into the layer under the skin, never into or beneath the muscle. A follow-up survey by Rios and Gupta, published in Aesthetic Surgery Journal in 2020, reported that the incidence of pulmonary fat embolism fell from 1 in 1,031 to 1 in 2,492 after that guidance spread. Ask any surgeon you consider how they confirm the injection plane.
Recovery: sitting, garments and time off
After a BBL, surgeons restrict sitting directly on the buttocks for a period so the grafted fat is not compressed; your surgeon sets the number of weeks. After implants, Santagostino describes a recovery phase of up to one week in a compression garment, which the surgeon may extend, and no physical exercise for at least a couple of months or until the surgeon clears it. Both Istanbul packages include a hospital admission: one night for a BBL, two nights for implants.
The date you can safely board a plane is a separate medical decision, not a package detail. We set out what governs it in our guide on when it is safe to fly home after a BBL. Patients from Milan and Rome face a flight of roughly three hours, which shortens the journey but does not replace the surgeon's clearance.
How to choose: five questions to put to the surgeon
First, ask how much fat the surgeon estimates you can donate, and what volume that gives each buttock. Second, ask whether they would recommend implants instead, and why. Third, for a BBL, ask how they keep the injection under the skin; for implants, ask which pocket they use and which implant brand and size they propose. Fourth, ask for the name of the hospital where you will be admitted. Fifth, check that the facility holds the Turkish Ministry of Health's international health-tourism authorisation. Our checklist on how to vet a BBL surgeon in Turkey covers credentials and red flags in more detail.
The short answer
If you have spare fat and also want a slimmer waist, a BBL from €4,050 is usually the better fit and, in Istanbul, the cheaper one. If you are lean, want a fixed volume or lost graft volume after a previous BBL, implants from €6,720 are the realistic option, but Italian patients should know that Milan's implant prices start lower than Istanbul's. A free video call with the assistant is the first step; a €30 video consultation with the surgeon, credited on booking, is the first paid step after you receive an offer. The surgeon, not the brochure, decides which operation you are a candidate for.
Frequently Asked Questions
Can I combine a BBL with butt implants?
Some surgeons combine an implant with fat transfer around it for a softer contour, a technique known as composite gluteal augmentation. Whether it suits you depends on your tissue and on the surgeon's experience with the technique. It is quoted individually rather than as a standard package: the BBL package from €4,050 and the implant package from €6,720 are each priced as a single procedure.
Are butt implants permanent?
Silicone gluteal implants are designed to stay in place long term, but no implant is guaranteed for life. The systematic review presented at the 2014 ASPS meeting found wound dehiscence in 10.3% and seroma in 4.9% of implant patients, and some complications lead to removal or revision surgery. Ask the surgeon which implant manufacturer they use, what its warranty covers and what the package's revision terms include.
Is a BBL result permanent?
Fat cells that survive the transfer behave like the rest of your body fat: they stay, and they grow or shrink with your weight. Part of the transferred fat does not survive, which is why the final shape settles over the following months. Keeping a stable weight after surgery protects the result, while large weight loss reduces the buttock volume as well as the rest of your body.
Which recovery is harder, a BBL or butt implants?
They are hard in different ways. A BBL adds bruising and swelling in the liposuction areas, and surgeons restrict sitting directly on the buttocks for a period. Implants need an incision and a pocket for the device, and Santagostino advises no physical exercise for at least a couple of months. Both are performed under general anaesthesia, and pain is managed with medication the surgeon prescribes.
How long do I need to stay in Istanbul for a BBL or butt implants?
The BBL package includes one hospital night and the implant package two, followed by hotel nights at the Deniz. The surgeon decides when you are fit to fly after a check-up. If you need to stay longer, extra nights at the Deniz cost €70 each, and an upgrade to the Antwell costs €110 per night. Flights are never included, so book a flexible return ticket.
