Lens Replacement (RLE) Recovery Week by Week: What to Expect

Most people see better 1 to 3 days after lens replacement, use eye drops for about 4 weeks, and reach their full visual result within 3 to 10 weeks. In Istanbul, both eyes are treated on two separate days inside a 4-night package, so you fly home with both eyes operated and checked. Below is what each stage looks like, with the source for every milestone.
Refractive lens exchange (RLE) uses the same technique as cataract surgery: the natural lens is removed through a small incision and replaced with an artificial intraocular lens, often a trifocal or EDOF lens designed to reduce dependence on glasses at several distances. For that reason the recovery milestones below come from public cataract-aftercare guidance published by the NHS and by Alberta Health Services. Your own surgeon's instructions always take priority over any general timeline.
Before you fly: the 4-night plan in Istanbul
The lens replacement package in Istanbul starts from €3,500 for both eyes and includes 4 hotel nights and airport transfers; flights are never included. From Milano Malpensa (MXP), Bergamo (BGY) or Roma Fiumicino (FCO), the direct flight to Istanbul Airport (IST) takes about 3 hours. The usual order is arrival and pre-operative eye measurements, the first eye, the second eye on a separate day, then a check by the partner ophthalmologist before you leave.

Each eye takes about 15 to 20 minutes under anaesthetic eye drops with light sedation, and you return to the hotel the same day. Your vision stays blurred for a few hours afterwards, mainly because of the pupil-widening drops, according to the NHS. You will not drive in Istanbul: the package transfers take you between hospital and hotel.
Days 1 to 3: blurred at first, then clearer
The NHS says the eye may hurt for a few days and that it can take several days for vision to improve. Alberta Health Services reports that most people see better 1 to 3 days after surgery and return to work or their normal routine within 1 to 3 days. You may be asked to wear a plastic shield at night while the eye heals, for as long as your surgeon advises.
Alberta Health Services says you can shower and wash your hair from the day after surgery, keeping water and shampoo out of the eye, especially in the first week, and that you should not rub or press on the eye for at least 1 week. The NHS puts the eye-drop course at about 4 weeks, so most of it happens once you are home.
Week 1: flying home and settling in
Neither the NHS nor Alberta Health Services sets a fixed no-fly period after lens surgery; what decides your departure is the post-operative check, where the partner ophthalmologist examines both eyes and confirms you are fit to travel. Keep your prescribed drops in your hand luggage so the schedule continues on the plane. Our guide to flying after laser eye surgery covers the airport and in-flight side in more detail.
Back home, avoid eye make-up for 1 to 2 weeks, and avoid swimming, hot tubs, gardening and dusting for 1 to 2 weeks (Alberta Health Services). The NHS is stricter on water: no swimming, even with goggles, and no water in the eye.
Weeks 2 to 4: drops, exercise and a local check
Avoid bending and strenuous exercise for 2 weeks or until your doctor clears you, and skip hair colouring or perms for 10 days (Alberta Health Services). The drop course ends around week 4. Book an in-person check with an ophthalmologist or optometrist at home during this window, and ask for your operation report and lens details at discharge so they know exactly what was implanted.
LASIK and SMILE reshape the cornea and keep your natural lens, while lens exchange replaces it. If you are still choosing between the two approaches, our comparison of LASIK and SMILE in Istanbul explains which patients suit a laser procedure instead of a lens exchange.
Weeks 4 to 10: judging the final result
Alberta Health Services puts the full benefit of the surgery at 3 to 10 weeks, so judge your reading and night vision at the end of that window rather than in week one. The NHS notes that some people still need glasses for reading or for distance after lens surgery, and that the prescription may differ from before. Wear sunglasses on bright days for at least 1 year.
Laser treatment starts from €1,800 in Istanbul against €3,500 for lens replacement; if cost is part of your decision, see our laser eye surgery cost comparison for 2026.
Warning signs: when to get help the same day
Contact your surgeon or a local eye emergency service the same day for eye pain, pus draining from the eye, worsening vision, redness that persists after 3 or 4 days, or new floaters, flashes of light or changes in your field of vision (Alberta Health Services). The last group matters most, because it can signal a retinal tear or detachment, which needs urgent treatment.
Retinal detachment is a recognised longer-term risk of lens surgery rather than a week-one problem. A 2017 ESCRS EuroTimes report on a session by Dr Oliver Findl identified young, short-sighted, male patients under 60 as the higher-risk group and said most detachments occur within five years of surgery. Ask your surgeon to assess your retina before the operation, especially if you are short-sighted.
Who follows you up after you fly home
Your Estetica assistant stays in touch after you leave and passes your questions about drops, symptoms or vision to the partner surgeon. Any urgent symptom should be seen in person by an eye service near you the same day; do not wait for a video call. Before booking, a free video call with our assistant covers the plan, and a €30 video call with the surgeon is the first paid step after your offer, credited if you book.
Frequently Asked Questions
Can both eyes be treated on the same day?
Not in this package. The NHS notes that both eyes are treated on separate days, and in Istanbul the partner ophthalmologist operates on the first eye, then on the second eye on a separate day, within the same 4-night stay. Treating the eyes one at a time lets the surgeon check the first eye before operating on the second, and you still fly home with both done.
When can I drive after lens replacement surgery?
The NHS advises not driving until your vision is back to normal and gives no fixed number of days, so the decision rests with the surgeon or the eye specialist who checks you at home. You also need to meet your country's legal eyesight standard. In Istanbul you will not need to drive, because the package transfers take you between hotel and hospital.
Will I still need glasses after refractive lens exchange?
Possibly for some tasks. The NHS notes that some people still need glasses for reading or for distance after lens surgery and that the prescription can change. Trifocal and EDOF lenses aim to reduce dependence on glasses at several distances, but results vary. Judge your vision once the full benefit has developed, which Alberta Health Services puts at 3 to 10 weeks.
Does lens replacement surgery hurt?
Each eye is treated under anaesthetic eye drops with light sedation and takes about 15 to 20 minutes. Afterwards the NHS says the eye may hurt for a few days and vision is blurred for a few hours from the pupil-widening drops. Worsening pain is different: Alberta Health Services lists it as a reason to contact your surgeon or a local eye service the same day.
Who has a higher risk of retinal detachment after lens replacement?
A 2017 ESCRS EuroTimes report on a session by Dr Oliver Findl named young, short-sighted, male patients under 60 as the higher-risk group, and said most detachments occur within five years of surgery. If you fit that profile, ask the surgeon to examine your retina before the operation and learn the warning signs: new floaters, flashes of light or changes in your field of vision.
