2026 prices: Istanbul, UK, US and Germany

Istanbul: from €17,920. Estetica Istanbul is an agency, not a clinic. The operation is performed by partner surgeon Dr. Ali Gürlek at Medivia Hospital in Çengelköy, using penile-inversion vaginoplasty. The package lists five hospital nights and six nights at the Deniz Hotel in Kadıköy, about eleven days in total, plus pre-operative tests, the anaesthesia consultation, post-operative medication, VIP airport transfers and follow-up checks. United Kingdom: from £31,160. The London Transgender Clinic's published price list starts its penile scrotal flap gender confirmation surgery at £31,160 and its colovaginoplasty at £34,310. At an illustrative rate of £1 = €1.15, £31,160 is about €35,830. The clinic says a formal quotation includes the hospital stay where required, and that guide prices can change after consultation.

United States: $23,000–24,500 plus fees. The Gender Confirmation Center in San Francisco says vaginoplasty patients are quoted $23,000–24,500, including the dilator kit but excluding anaesthesia, hospital facility fees and pathology. At an illustrative rate of $1 = €0.86, that is about €19,780–21,070 before those extras. A follow-up labiaplasty, done about five months later, is quoted separately at $8,500–15,500.

Germany: usually insurance, otherwise €8,000–15,000. Statutory insurers generally pay for gender-affirming surgery when the patient meets their conditions, according to the German insurance portal finanzen.de (last updated June 2023): two specialist reports, at least one year of psychotherapy and one year living in the affirmed gender. For self-payers, the price portal Estheticon.de lists total treatment costs of €8,000–15,000, based on a single doctor's listing, so treat it as a rough marker.

Istanbul vs Italy: Rome's private price and what it leaves out

In Italy, Dr. Patrizio Vicini, a plastic surgeon in Rome, states on his vaginoplasty page that MtF surgery costs about €15,000–17,000, with a hospital stay of at least four to six days. The page does not say whether the hospital, anaesthesia or follow-up are inside that figure. Istanbul's €17,920 sits €920 above the top of that range and lists the hospital nights, the hotel nights and the transfers in writing.

The legal route differs too. Vicini's page notes that in Italy this surgery requires prior court authorisation under Law 164 of 1982. A patient flying from Milano Malpensa or Roma Fiumicino to Istanbul, about three hours, pays for the flight separately. In Istanbul the operation follows the partner surgeon's medical criteria, and you bring your own documentation.

The Italian price guide Guidaestetica.it gives an average of €9,000 for gender-affirming surgery, but that figure mixes procedures of very different size, with simpler operations starting at €1,500–2,000. It is not a like-for-like vaginoplasty price. For a single vaginoplasty, the Rome quote is the closer comparison.

What the Istanbul package includes, and what you pay on top

Included in the €17,920: surgery, anaesthesia, five hospital nights, six recovery-hotel nights at the Deniz Hotel, pre-operative tests, medication, support garments where required, VIP airport transfers, a patient coordinator on WhatsApp, follow-up checks in Istanbul and remote follow-up after you fly home. The Deniz Hotel is priced at €70 per night; choosing the Antwell instead costs €110 per night.

Not included: flights, a companion's flights, and any nights beyond the planned eleven days. The deposit is €500. If you cancel at least 14 days before surgery and before your flight is booked, it is refunded; otherwise it becomes a 12-month credit. The first video call with your assistant is free; a video call with the surgeon costs €30 and is credited when you book.

Medical readiness: what the surgeon asks for

According to the procedure page, the partner surgical team follows the WPATH Standards of Care, version 8 (2022). SOC8 recommends hormone therapy before genital surgery unless it is not wanted or is medically contraindicated, and it asks for a documented assessment by a qualified health professional. Estetica does not provide hormone therapy, assessment letters or legal gender recognition. You bring your own prescription history and letter; your coordinator confirms what the surgeon needs before you pay anything.

Vaginoplasty is major surgery under general anaesthesia, lasting four to six hours according to the procedure page, and it carries real risks. Bleeding, wound separation, narrowing of the canal and urinary problems are complications to discuss with the surgeon at consultation. Dilation is part of recovery and continues at home, so ask how it is supervised and how follow-up works after you fly home before you book.
