Trans Patients in Istanbul: Privacy, Legal Status and Medical Readiness (2026)

Trans patients from abroad can have gender-affirming surgery in Istanbul with partner surgeons, but three things differ from home: legal status, data privacy and medical documents. Legally, Türkiye scores 5% on ILGA-Europe's Rainbow Map 2026 and ranks 47th of 49 countries, so plan for discretion in public rather than legal protection. Your medical file falls under stricter rules: Turkish data-protection law treats health data as a special category. Medically, partner surgeons work to the WPATH Standards of Care, Version 8 (SOC8): if a letter is needed, one letter from a qualified health care professional is enough. You bring your own documentation; Estetica Istanbul does not issue it.
What the Rainbow Map 2026 says about Türkiye
ILGA-Europe's Rainbow Map 2026 scores 49 European countries on laws and policies that affect LGBTI people. Türkiye kept 47th place with a score of 5%; Italy, for comparison, ranked 36th. Same-sex relationships are not a crime in Türkiye, but Turkish law gives no protection against discrimination based on gender identity, and the Istanbul governor's office has banned the city's Pride march every year since 2015.
That legal picture affects a medical trip less than the ranking suggests, because most of the stay is spent between the hospital, the hotel and private transfers. Many trans patients still choose discretion in public, travel with a companion and stay away from demonstrations. Our guide to flying home after FFS and trans surgery explains how long that stay usually lasts.
Your passport, your name and Turkish law
Article 40 of the Turkish Civil Code governs legal gender change for people in the Turkish civil register. The applicant must be over 18 and unmarried, needs an official health-board report from a training and research hospital, and needs a court's permission. It is a procedure for Turkish records. It does not change a foreign passport, and legal gender recognition stays a matter for your own country. Neither Estetica Istanbul nor any partner hospital arranges it.
Book flights, the hotel and hospital admission in the name printed in your passport, because airlines and hospitals check identity against it. If that name or gender marker differs from how you live, tell your assistant at the first contact. The booking then uses your passport name, and the team uses your chosen name and pronouns when talking to you. Ask the partner hospital in writing which identity and medical documents it needs from a foreign patient before you buy a ticket.
Who sees your medical file
Türkiye's Personal Data Protection Law No. 6698, known as KVKK, lists health data and data on sexual life as special categories of personal data in Article 6. Special-category data may be processed only under stricter conditions than ordinary personal data. Your hospital file falls under that law.
Before you send photos, ask who will see them: as an agency, Estetica Istanbul passes your photos and forms to the partner surgeon who assesses you. Say in writing whether you agree to before-and-after use of your photos. You can refuse, and refusing does not change the offer.
Medical readiness under WPATH SOC8
WPATH published SOC8 in 2022. It lists the criteria for adults seeking gender-affirming surgery: gender incongruence that is marked and sustained; capacity to consent to the specific procedure; an understanding of the effect on reproduction, with options explored; other possible causes of the incongruence excluded; mental and physical conditions that could affect the outcome assessed; and stability on any hormone regimen.
SOC8 changed two rules that older clinic checklists still repeat. For genital surgery, it no longer recommends two referral letters: if a letter is needed, one letter from a qualified health care professional is enough. It also dropped the requirement of 12 months living in your gender role. If hormones are prescribed, SOC8 suggests at least 6 months on a stable regimen before genital surgery, unless hormones are not wanted or are medically contraindicated.
Each partner surgeon decides which documents a procedure needs. You bring your own letter, hormone history and medical records; Estetica Istanbul does not prescribe hormones, write psychological letters or arrange legal recognition. If your letter is not in English or Turkish, ask whether a certified translation is needed. The checks for choosing an FFS surgeon in Istanbul include the questions to ask about documents.
Five questions to ask before you pay the deposit
Put these to the partner surgeon in writing. First, which SOC8 documents does the hospital require for your procedure? Second, must a letter be dated within a set period before surgery? Third, how will your passport name and your chosen name appear on the hospital file and the invoice? Fourth, who can see your photos and records? Fifth, what happens to your booking if the surgeon postpones surgery after the in-person exam?
Also check that the hospital holds the Turkish Ministry of Health's international health tourism authorization. The free video call with the assistant comes first. After the offer, a €30 video call with the partner surgeon is the first paid step, credited if you book.
Prices for the main procedures
Through Estetica Istanbul, male-to-female genital surgery with a partner surgeon starts from €17,920, facial feminization surgery from €10,060 and FtM top surgery from €3,900. Each price is a package covering the surgeon's fee and hotel nights at the Deniz hotel. Flights are never included.
The €500 deposit secures the surgery date. Cancel at least 14 days before surgery and before your flight is booked, and it is refunded; otherwise it becomes a 12-month credit. Our MtF vaginoplasty cost comparison sets the €17,920 package against prices in the UK, the US, Germany and Italy.
Travelling from Italy and the rest of Europe
Direct flights from Milano Malpensa (MXP) and Roma Fiumicino (FCO) to Istanbul Airport (IST), and from Milano Bergamo (BGY) to Sabiha Gökçen (SAW), take about three hours. Carry your letter, your prescriptions and your medical records in hand luggage, and keep hormone medication in its original packaging with a copy of the prescription. Ask the surgeon whether and when to pause hormones around surgery; the instruction is individual.
Read your travel insurance before you book: many standard policies exclude planned surgery abroad and its complications. Ask about specialist cover for medical travel, and keep the assistant's number and your operative notes with you on the flight home.
Frequently Asked Questions
Do I need two letters for vaginoplasty in Istanbul?
Under WPATH SOC8, no. SOC8 replaced the older two-letter recommendation for genital surgery: if a letter is needed, one letter from a qualified health care professional is enough. The partner surgeon decides which documents your procedure requires, so ask in writing before you book. You bring your own letter and records; Estetica Istanbul does not write psychological letters or prescribe hormones.
Will my passport show my new gender after surgery in Istanbul?
No. Article 40 of the Turkish Civil Code covers legal gender change only for people in the Turkish civil register, through a Turkish court. It does not change a foreign passport. Legal gender recognition is handled by your own country under its own rules, and neither Estetica Istanbul nor any partner hospital arranges it. Book flights, hotel and hospital admission in your current passport name.
Can I bring my hormone medication to Türkiye?
Carry hormone medication in hand luggage, in its original packaging, with a copy of the prescription, and bring your letter and medical records in the same bag. Take enough for the whole stay plus a few spare days in case your flight changes. Ask the partner surgeon whether and when to pause hormones around surgery, because that instruction is individual.
Is Istanbul safe for trans patients during recovery?
No city can promise that. ILGA-Europe's Rainbow Map 2026 ranks Türkiye 47th of 49 countries with a score of 5%, and Turkish law gives no protection against discrimination based on gender identity. Most of a medical stay is spent in the hospital, the hotel and private transfers. Many patients travel with a companion and keep a low profile in public.
Do I need a certified translation of my SOC8 letter?
Ask the partner surgeon which languages the hospital accepts before you book. A letter in English or Turkish is usually read as it is; a letter in another language may need a certified translation, which you arrange and bring with the original. Under SOC8, one letter from a qualified health care professional is enough if a letter is needed at all.
