What lipedema surgery involves

Lipedema is a chronic disorder of fat distribution, almost exclusively in women, in which disproportionate, painful fat accumulates on the legs and often the arms, resists diet and exercise, and bruises easily. It is not obesity and it does not respond to weight loss. Conservative treatment — compression, manual lymphatic drainage — manages symptoms; surgery is the only treatment that removes the diseased tissue.

The operation is lymph-sparing liposuction: water-jet (WAL) or power-assisted (PAL) liposuction with fine cannulas in the direction of the lymph vessels, under tumescent local or general anaesthesia, removing three to six litres of lipedema fat per session. Because of the volume involved, treatment is staged: typically outer thighs and knees first, inner thighs and lower legs second, arms third, with eight to twelve weeks between sessions. Most patients report a marked drop in pain and heaviness within weeks of the first stage.

Who is eligible

A diagnosis of lipedema from a phlebologist, lymphologist or dermatologist, stable weight, and a BMI the surgical team is comfortable operating on (usually under 35; higher with a plan). Bring your diagnosis letter, any lymph-scintigraphy or ultrasound, and your compression prescription. Patients with active lymphoedema or venous disease are assessed first. Smoking stops four weeks before surgery.

Why Istanbul, and why through us

In Germany, the Netherlands and the UK, insurers cover lipedema surgery only in narrow cases and private clinics charge €8,000–15,000 per stage with months of waiting. Our board-certified partner plastic surgeons perform high-volume VASER and WAL liposuction in a JCI-accredited private hospital, and the same lymph-sparing technique costs a third of the price with a date within weeks. Compression garments are fitted before you fly home and your coordinator speaks German, Dutch or English.

You deal with one coordinator, one hospital and one medical team from the first message to the last follow-up. Every price is itemised in writing before you pay a deposit, and the written quote does not change unless you change the plan. Flights are never included.

Recovery and follow-up

One night in hospital, then the recovery hotel with daily checks; most patients fly home on day six or seven. Compression is worn day and night for four weeks, then daytime for four more. Walking from day one, desk work at one to two weeks, sport at six weeks. Manual lymphatic drainage at home in the first weeks speeds the result. Stages two and three are booked at your pace, usually eight to twelve weeks apart, with the same surgeon and the same coordinator.

Cost of lipedema surgery in Turkey

The published price is per stage and includes one hospital night, recovery hotel, transfers, compression garments and coordination. The number of stages depends on the volume to be removed and is set at consultation, usually one to three. Multi-stage plans are quoted together at a reduced total. Flights are never included.

Begin with a confirmed assessment, not a body-shaping package

If you are seeking help for suspected lipedema, explain your symptoms, previous assessments and current management. Ask who confirms the diagnosis and whether another condition could explain your concerns. Do not infer a diagnosis from a photograph or assume that every leg-contouring procedure treats the same problem.

Describe the effect on your daily life in your own words. Ask which goals the proposed treatment addresses and how improvement would be assessed. A cosmetic photograph is not enough to explain a plan intended to address symptoms or function.

Discuss the evidence and uncertainty directly

Ask the specialist to explain the evidence for the proposed approach, its risks and how it compares with appropriate non-operative management.

The NICE guidance on liposuction for chronic lipoedema should be read in its UK context. Do not convert a guidance reference into a claim that surgery is guaranteed to cure the condition or that the named Turkish provider follows a particular protocol.

Ask whether the proposal is one stage or several

Request the areas planned for treatment, the reason for that selection and whether later procedures are anticipated. Ask what would influence the timing and decision to proceed to another stage.

A package number should not conceal several trips. Ask for the cost of each proposed stage, the accommodation covered and any additional services or equipment you need to arrange. Flights are excluded.

Continue the care plan beyond the operation

Ask which existing management should continue, which may change and who supervises those decisions. Do not change compression, medication or other prescribed care based on a generic article.

If support will be provided at home, discuss that arrangement before travelling. Obtain records of the assessment and treatment plan that another clinician can use.

Is treatment mainly about appearance?

Explain your own priorities and ask the specialist how symptom, function and appearance goals are assessed separately.

Does surgery replace all ongoing management?

Do not assume that. Ask for a personalised plan and its uncertainties.

What should the quote make clear?

The treated areas, number of stages, named clinicians, included services and the possible need for further travel or ongoing care.

Record symptom goals separately from treatment areas

Bring previous diagnostic letters, test reports if available, your compression prescription and details of current care. Describe pain, heaviness, bruising and the activities you find difficult, including how these have changed. Ask which symptoms the proposed procedure is intended to address and which may have another cause.

Request a plan for assessing benefit beyond photographs: how you feel and function, what remains uncertain and when reassessment occurs. An operation that changes contour should not automatically be described as resolving every symptom. Ask how the specialist will distinguish a treatment limitation from a concern needing further investigation.

Keep non-operative care in the comparison

Ask what changes to your existing management could help, whether further specialist assessment is useful and whether waiting is reasonable. Discuss compression, activity support and other care with the clinician responsible for your condition; do not replace that plan with a cosmetic-liposuction package.

Ask the specialist to explain the evidence limitations and rationale for surgery. Discuss NICE's lipedema guidance alongside the proposed plan; its existence does not establish an individual benefit or a partner protocol. Ask what remains uncertain rather than accepting a cure or complete-symptom-relief promise.

Make each stage a fresh clinical decision

Ask for the proposed areas at each stage and why they are grouped that way. Clarify which findings after the first stage could change the next operation, its timing or the recommendation to proceed. A multi-stage discount should not commit you to later surgery before reassessment.

The catalogue's €4,900 entry is a per-stage market benchmark with a one-night hospital notation. It does not establish the number of stages, total hotel nights or a confirmed partner package. Request a current estimate for the whole proposed sequence and each stage separately, identifying garments, follow-up, tests, accommodation and extras. Each return trip needs its own flight budget.

Arrange support at home before the first trip

Ask who supervises compression fitting, wound care and any continuing therapy after return. If existing local care is involved, share the proposed treatment plan and check whether appointments are available. Do not assume that massage or manual drainage is included, required for everyone or interchangeable with specialist follow-up.

Does “lymph-sparing” mean there is no lymphatic risk?

Ask the clinician to explain the technique's intended purpose and remaining risks, including swelling, healing problems and lymphatic injury. The label is not a guarantee. Obtain instructions for concerning changes and the location of urgent assessment.

03

Partner healthcare institutions

Your operation takes place inside a fully accredited hospital — dedicated anaesthesia and nursing teams, intensive care on the same floor, international safety standards. Never a back-street day-clinic.

04

Specialist Clinics for Smile & Hair

Non-surgical treatments happen at purpose-built venues — our dedicated partner dental clinic for veneers, crowns and implants, and our partner hair restoration clinic for transplants. Each run by teams who do only that, every day.

05

Recovery, Follow-Up & Your Smile

You recover in a hotel with on-site medical care, then fly home. We follow up remotely for a full year — and if a complication arises, we cover remote consultations with your surgeon and, in rare cases, revision surgery — with the Antwell upgrade.

Frequently Asked Questions

How many stages will I need?
Most patients need two; stage I lipedema on the legs alone is often one session, extensive leg-and-arm disease three. The surgeon sets the plan from your photos and measurements at consultation, before you pay a deposit.
Is it lymph-sparing?
Yes. Fine cannulas are used longitudinally along the lymph vessels with WAL or PAL technique, which is the standard of care for lipedema and the reason ordinary cosmetic liposuction is not appropriate.
Will my insurer reimburse it?
Usually not; that is why most patients come. We provide an itemised invoice and an operative report for any claim you wish to make.
Does the fat come back?
Lipedema fat cells that are removed do not return. Remaining tissue can still progress, which is why weight stability and compression matter after surgery.
