Forehead Reduction

Forehead reduction — also called hairline lowering — shortens a tall or disproportionate forehead by advancing the hairline and removing a strip of upper-forehead skin. For the right patient it reframes the whole face in a single operation, something no amount of styling can achieve on its own.

Who forehead reduction is for

It suits patients with a naturally high hairline, a forehead that looks long in proportion to the mid- and lower-face, or a strong, stable hairline simply set too far back. Candidates need good hair density at the front, since the technique advances existing hair rather than transplanting it. Where the hairline is thinning, we may recommend combining it with, or replacing it by, a hair transplant.

The procedure

Through an incision placed precisely along the hairline, the scalp is advanced forward and the excess forehead skin removed, lowering the hairline by one to two-and-a-half centimetres in a single stage. The operation takes around 1.5 to 2 hours under general anaesthesia, with one overnight stay. The scar sits at the hairline and is quickly camouflaged by the hair growing through and in front of it.

Honest expectations

The trade-off is a fine hairline scar. In most patients it fades to a barely visible line hidden by the front hair, but healing varies and we plan the incision honestly on that basis. Scalp numbness behind the scar is normal for a few months. Patients wanting more than about 2.5 cm of change, or with a weak frontal hairline, are usually better served by a hair transplant, and we will say so.

Discuss the hairline you want to preserve

Before focusing on how far to lower the hairline, explain which features you like about its existing shape. Ask the surgeon to discuss the proposed line from the front and at the temples, and how it would look with the hairstyles you normally wear. A distance in centimetres does not describe the whole result.

Tell the clinician about changes in hair density, previous scalp procedures and your hair-loss history. Ask whether those factors affect the recommendation or whether a separate hair assessment is needed.

Compare hairline lowering with other approaches

Ask how forehead reduction differs from a transplant-based approach for your situation. The comparison should address the proposed change, limitations, scar placement, appointments and future planning.

You can also postpone treatment. If you are unsure about your preferred hairline or the trade-off involved, ask for time and a clear explanation before agreeing to surgery. A small change deserves the same informed decision as a larger one.

Consider the scar before accepting the plan

Ask to see where the incision would be placed on your own hairline. Discuss how the scar may relate to hair styling and what uncertainty exists around its appearance. Avoid choosing surgery on the assumption that a scar is guaranteed to disappear or always remain concealed.

If another procedure is proposed later to address the scar or hairline, ask whether that is a possibility, a recommendation or part of the agreed plan. Request its own assessment and cost rather than treating it as an automatic included adjustment.

Prepare for the stay and follow-up

Ask how to plan washing, grooming, headwear and your return to work. Request instructions from the treating surgeon rather than copying another patient's routine.

The written quote should identify the operation, hotel nights and follow-up arrangements. Flights are not included, and the package length is separate from clinical clearance to travel.

Can I specify an exact amount of lowering?

Explain the change you want and ask what the surgeon considers feasible for your anatomy. A requested number is not an outcome guarantee.

Is hair transplantation always an alternative?

Ask for an individual comparison and assessment; do not infer suitability from photographs alone.

Will my scar be invisible?

Ask for realistic expectations and comparable cases. Do not book on an absolute promise about scar appearance.

Plan for possible changes in the surrounding hair

A lowered hairline should be considered alongside the hair you may retain in future. Explain thinning at the temples or crown, family history, previous transplants and treatment for hair loss. Ask how the surgeon assesses scalp movement and whether advancing existing hair is appropriate. A photograph cannot establish every factor governing the amount of lowering.

Ask the surgeon to mark the proposed line and explain its shape with your forehead muscles relaxed. Clarify whether any brow procedure is also being proposed: changing hairline position and changing brow position are different goals. If a combination is suggested, ask for separate benefits, risks and costs instead of assuming one operation includes the other.

Discuss hair loss and further treatment as possible trade-offs

Ask about temporary hair loss, haematoma, scar revision and the possibility of additional treatment. If discussing this US single-surgeon hairline-lowering study, ask how its technique and patient group differ from your proposed operation. Do not use its results as a prediction for an Istanbul partner.

Discuss altered sensation, healing problems and the possibility that a scar remains visible with your usual hairstyle. Ask what assessment would precede any later scar procedure or transplant. Further treatment is a possibility to discuss, not a promised included finishing step.

Make the quote distinguish lowering from later refinement

As of 8 October 2026, the published price list lists forehead reduction from €3,550 with five package nights. The written quote should confirm the partner, operation, accommodation and any combined procedure. Flights are excluded. Ask whether a proposed future transplant would involve another assessment, another trip and a separate charge.

Ask which checks take place before leaving Istanbul and how wound care, hair washing and return to headwear are handled afterwards. The five-night allowance does not set the date for suture removal or flight clearance. Have the surgeon specify those decisions for your own wound and journey.

Is a brow lift an alternative to hairline lowering?
