Refractive lens exchange (RLE) with trifocal lenses costs from €3,500 for both eyes in Istanbul through Estetica Istanbul's partner eye hospital, all-inclusive except flights. The same operation is priced from £3,495 per eye (£6,990 for both) with a multifocal lens at Optical Express in the UK, $4,000 to $10,000 per eye in the US according to Brinton Vision, €2,900 to €4,400 per eye with a trifocal lens in Germany according to myBody.de, and €2,900 to €3,300 per eye with a multifocal lens at Florence Medical Center in Italy. Below: where each number comes from and what it buys.
What €3,500 buys in Istanbul
The Istanbul price covers both eyes with premium trifocal lenses. It includes the diagnostic work-up on arrival (biometry, corneal topography and a retinal OCT scan), surgery on both eyes, eye drops, hotel nights, airport and hospital transfers, and a coordinator who speaks your language. Toric lenses for astigmatism and extended-depth-of-focus (EDOF) lenses are quoted individually; monofocal lenses cost less. Flights are never included. The full list of inclusions is on the lens replacement surgery in Istanbul page.

Estetica Istanbul is a medical-tourism agency, not a clinic. The operation is performed by a partner ophthalmologist at a JCI-accredited dedicated eye hospital in Istanbul, with lenses from the manufacturers used across Western Europe: Alcon, Zeiss and Johnson & Johnson. The standard itinerary is four nights: diagnostics on day one, the first eye on day two, the second eye on day three, then a check and the flight home on day four. Our week-by-week guide to lens replacement recovery covers what happens after you land.
Lens replacement cost by country in 2026
United Kingdom: Optical Express lists lens replacement from £2,000 per eye with a monofocal lens and from £3,495 per eye with a multifocal lens, so £6,990 for both eyes at the multifocal entry price. Its price page states that the final figure depends on your prescription and is set at consultation. Aftercare appointments and prescribed drops for the first three months are included; after that, an optometrist visit costs £100 and a surgeon consultation £200.
United States: Brinton Vision, a refractive surgery practice in St. Louis, puts the average cost of refractive lens exchange at $4,000 to $10,000 per eye, which is $8,000 to $20,000 for both eyes. The practice points patients to payment plans and to HSA, FSA or HRA funds rather than to insurance.
Germany: the price guide published by myBody.de lists trifocal lens exchange at €2,900 to €4,400 per eye, or €5,800 to €8,800 for both eyes, and multifocal lenses at €2,500 to €3,700 per eye. A femtosecond-laser-assisted operation adds €1,000 to €1,300 per eye. AugenCentrum Hamburg West advertises trifocal, multifocal and EDOF lenses from €2,750 per eye. myBody.de describes refractive lens surgery as a self-pay service and notes that statutory insurers do not cover premium lenses.
Lens replacement in Istanbul vs Milano and the rest of Italy
Italian clinics publish their prices more openly than most. Florence Medical Center in Florence lists cataract surgery with a multifocal lens at €2,900 to €3,300 per eye depending on technique, which is €5,800 to €6,600 for both eyes; its €150 pre-operative visit is deducted from the surgery cost. Centro Oculistico Sardo lists a premium multifocal lens operation at €3,300 for one eye.
Closer to Milano, Gruppo Refrattivo Italiano in Brescia and Bergamo lists femtosecond-laser cataract surgery from €3,400 for one eye and €6,600 for both eyes with a monofocal lens, and up to €8,600 with premium toric or EDOF lenses. Six months of check-ups are included, and the pre-operative visit costs €150 extra. Against those figures, the €3,500 Istanbul package for both eyes with trifocal lenses is €2,300 below the lowest Italian two-eye price above (€5,800) and €5,100 below the €8,600 premium-lens price, with hotel and transfers included.
The trade-off is travel. Direct flights from Milano Malpensa (MXP), Bergamo (BGY) and Roma Fiumicino (FCO) to Istanbul take about three hours, and the itinerary needs four nights in Istanbul. Flights are not in the package, so add the fare to the comparison before deciding.
Why the price gap exists
The gap comes from the cost base, not the lens. The implants come from the same manufacturers, but surgeon fees, hospital overheads and staff costs in Istanbul are lower than in London, Munich or Milano. Volume also lowers the price: a dedicated eye hospital that treats international patients every week spreads its fixed costs over more operations. The operation itself does not change: the natural lens is removed through a small incision with ultrasound and replaced with an artificial lens, the same procedure as cataract surgery.
What the foreign prices usually leave out
Compare like with like. Optical Express, Brinton Vision, myBody.de and Florence Medical Center quote per eye, so double the figure for both eyes. Gruppo Refrattivo Italiano charges the pre-operative visit separately, and Optical Express charges for visits after month three. None of the foreign figures includes a hotel. The Istanbul package includes hotel and transfers but not flights, and follow-up continues at home: the checks in the partner ophthalmologist's written aftercare schedule can be done by an optometrist, using the lens record you take with you.
Risks are the same in every country
Lens replacement is low-risk but not risk-free, wherever it is done. A large published series by Schallhorn and colleagues in the American Journal of Ophthalmology (2019) followed 18,689 eyes after refractive lens exchange: 6% had an adverse event, 0.9% had one rated serious and potentially sight-threatening, and the most common problem, clouding of the capsule behind the new lens (posterior capsule opacification), affected about 4% of eyes. Multifocal and trifocal lenses can also cause halos around lights at night. Ask the ophthalmologist how a complication would be handled once you are home, and get the answer in writing.
Who should choose a different procedure
Lens replacement is usually offered to adults with presbyopia (age-related loss of near focus), or to people whose prescription is outside the range of laser correction. It is not suitable for people with active macular disease, advanced glaucoma or uncontrolled diabetes. If you are younger and your only problem is short-sightedness or astigmatism, laser correction keeps your natural lens; our comparison of LASIK and SMILE in Istanbul explains those options.
How to book and what you pay first
Send your current prescription and any recent eye reports. The first video call with your Estetica assistant is free. After you receive the offer, the first paid step is a €30 video call with the specialist, credited if you book. A €500 deposit secures the date: cancel at least 14 days before surgery and before the flight is booked and it is refunded; otherwise it becomes a 12-month credit. The lens is chosen only after the measurements in Istanbul, so the written quote names the lens model and confirms that it covers both eyes.
Frequently Asked Questions
Is the €3,500 Istanbul price for one eye or both eyes?
It is for both eyes. The €3,500 package covers refractive lens exchange on both eyes with premium trifocal lenses, plus diagnostics on arrival, eye drops, hotel nights, transfers and a coordinator. Toric and EDOF lenses are quoted individually and monofocal lenses cost less. Flights are not included. Most foreign prices are quoted per eye, so double them before comparing.
Does health insurance pay for refractive lens exchange?
It depends on your country and policy. In Germany, the myBody.de price guide describes refractive lens surgery as a self-pay service and says statutory insurers do not cover premium lenses. In the US, Brinton Vision points patients to payment plans and HSA, FSA or HRA funds. Check your own policy in writing before you book anywhere; the Istanbul package is paid by the patient.
Can both eyes be operated on in the same trip to Istanbul?
Yes. The standard itinerary is four nights in Istanbul: diagnostics on day one, the first eye on day two, the second eye on day three, then a check and the flight home on day four. The partner ophthalmologist confirms the schedule after the measurements, and the second eye can be moved if the first needs more time.
Are the lenses used in Istanbul the same brands as in Italy or the UK?
The partner eye hospital uses lenses from Alcon, Zeiss and Johnson & Johnson, the manufacturers used across Western Europe. The exact model is chosen only after biometry, corneal topography and a retinal scan in Istanbul, and it is named in your written quote. Ask for the lens record before you fly home so any optometrist can see what was implanted.
How does follow-up work after I fly home?
Follow-up checks at home follow the partner ophthalmologist's written aftercare schedule and can be done by an optometrist, using the lens record you take with you. Bring the written instructions and your drops schedule. If something worries you, contact your Estetica assistant on WhatsApp, who connects you with the partner team; ask before surgery how an urgent problem would be assessed once you are home.
