Full Mouth Dental Restoration in Istanbul: All-on-4 vs All-on-6 Implants Explained
When a patient is missing most or all of their teeth, traditional tooth-by-tooth implant restoration can require 10-14 individual implants and years of treatment. All-on-4 and All-on-6 are revolutionary full-mouth dental restoration procedures that provide a complete new set of teeth supported by just 4-6 dental implants respectively. These innovative techniques have transformed the lives of thousands of patients worldwide, providing a rapid solution to complete tooth loss or the need to extract remaining damaged teeth. Istanbul has become a world-leading destination for All-on-4 and All-on-6 procedures, offering exceptional expertise, cutting-edge facilities, and all-inclusive packages that deliver extraordinary value compared to similar treatments in Europe and North America.

The All-on-4 procedure, pioneered in the 1990s, uses a strategic placement of 4 dental implants to support a complete arch of 12-14 teeth. Two implants are placed straight into the jaw near the front of the mouth, while the other two implants are angled toward the back (distal implants). This specific angulation allows the posterior implants to maximize bone contact despite previous bone loss, reducing or even eliminating the need for bone grafting. The angled placement distributes bite forces evenly across all four implants, providing exceptional stability and load capacity. A custom-fabricated bridge (prosthesis) containing a complete set of teeth is then anchored to these four implants, either through screw retention or cement retention.
The All-on-6 procedure represents an evolution of the All-on-4 concept, using six dental implants instead of four. These six implants are typically placed in a more parallel arrangement—four in positions similar to All-on-4, plus two additional implants. The extra implants provide enhanced stability, better load distribution, and a wider foundation for the prosthesis. All-on-6 procedures typically deliver superior long-term stability and can better withstand the significant bite forces that occur during normal chewing and eating. Patients with stronger bite forces, more demanding functional requirements, or those seeking maximum longevity often prefer the All-on-6 approach.
Determining whether you're a candidate for All-on-4 versus All-on-6 involves assessing your bone density, bone volume, bite force characteristics, and personal preferences. The advantage of All-on-4 is that it can often be performed with less bone grafting and sometimes even in patients who have experienced significant bone loss. All-on-6 typically requires adequate bone in more locations along the jaw but provides superior stability and longevity. At Estetica Istanbul, our experienced partner implant surgeons use advanced cone-beam CT imaging to assess your bone anatomy and recommend the approach best suited to your individual circumstances. In many cases, both approaches are viable, and the decision becomes a matter of personal preference and long-term vision for your restoration.
The €3,500-per-arch All-on-4 entry is a market benchmark, not a confirmed available partner package. Request a current quote identifying the arch, implant system, extractions, temporary and definitive restorations, diagnostics and each visit. All-on-6 requires its own assessed proposal. Compare current written provider quotes before calculating an overseas saving.
The timeline for All-on-4 and All-on-6 procedures varies based on whether you choose immediate loading or traditional delayed loading. With immediate loading, many patients receive their complete temporary restoration (fixed bridge with teeth) on the same day as implant surgery, providing immediate functionality and esthetics. You can return home to Europe with a complete set of teeth within 3-5 days. After 3-6 months of osseointegration, you return for your final permanent restoration, which is crafted to precise specifications and optimized for your bite, function, and long-term comfort. Delayed loading involves waiting 3-6 months after implant placement before the final bridge is fabricated, but some patients prefer this approach for maximum implant stability and longevity.
Recovery from All-on-4 or All-on-6 surgery follows a similar timeline to single implant placement, though the extent of surgery is greater. Most patients can return to light activities within 1-2 weeks and resume normal activities by week 4. With immediate loading, you can begin eating soft foods within 24-48 hours, gradually progressing to normal diet over several weeks. With a fixed prosthesis supporting your teeth, you'll enjoy superior functionality compared to dentures—no removal, better taste and sensation, and complete stability during eating and speaking. Pain is typically well-controlled with prescribed medication, and most patients report that discomfort is less than expected.
The longevity of All-on-4 and All-on-6 restorations is exceptional when properly maintained. The implants themselves, when successfully integrated with bone, can last 20-30 years or longer. The fixed prosthesis (bridge) typically has a lifespan of 10-15 years, after which it may need replacement or repair. This longevity is dramatically superior to removable dentures, which require replacement every 5-7 years as the jaw bone continues to resorb and the denture no longer fits properly. For All-on-4 or All-on-6 patients, this represents a true permanent solution to tooth loss—not a temporary fix requiring repeated replacement.
Living with All-on-4 or All-on-6 is remarkably similar to living with natural teeth. Your fixed prosthesis is cleaned like natural teeth—brushing twice daily with a soft-bristled toothbrush and daily flossing. You eat normal foods without restriction; unlike denture wearers, you don't need to avoid any foods or remove your teeth for cleaning. You speak naturally without the slurred speech sometimes experienced by denture wearers. You smile confidently knowing that your teeth won't slip or move during conversation or eating. Many patients report that within weeks of receiving their All-on-4 or All-on-6 restoration, they've completely forgotten that their teeth aren't natural.
At Estetica Istanbul, our comprehensive All-on-4 and All-on-6 programs include everything necessary for a successful restoration: advanced diagnostics and 3D surgical planning, experienced partner implant surgeons trained internationally, premium dental implants from leading manufacturers, a temporary restoration for immediate functionality, and a custom-fabricated final prosthesis. All follow-up care during the first year is included in your package price. International patients receive detailed guidance on pre-operative preparation, post-operative recovery management, and long-term maintenance. If you're considering a permanent solution to tooth loss or planning to extract damaged remaining teeth and require full-mouth restoration, All-on-4 and All-on-6 offer life-changing results. Clarify whether the plan concerns one arch or both
Ask the dentist to identify the arch being treated and what happens to teeth that are still present. “Full mouth” and “full arch” are different descriptions; they should not be used interchangeably in a quote. Request a written explanation of which teeth, implants and restorations are covered.
If extraction is proposed, ask why each tooth needs removal and what alternatives have been considered. A package advertised around an implant count should not determine whether an individual tooth is retained.
Distinguish the first restoration from the final one
Ask what you will leave Istanbul with after the initial procedure, whether it is temporary and which further appointments are anticipated. Request the timeline from the treating dentist rather than assuming that a same-day photograph shows completed treatment.
The ADA implant guide distinguishes implants from the crowns, bridges or dentures they support. Your quote should make those components and stages visible, including what is included in the price and what needs a later agreement.
Compare four and six implants through the clinical rationale
Ask the dentist why the proposed number and positions are appropriate for your plan. Do not assume that a higher count is always better, or that a lower count establishes the same treatment for every patient.
Discuss the alternatives, the intended benefit and the material risks before choosing a fixed restoration. If several proposals sound equally plausible, ask for a clear explanation of the trade-offs or seek another opinion.
Do not treat a benchmark as a bookable package
The price list's All-on-4 figure is labelled as a market benchmark rather than a confirmed partner price. Obtain a current quote for the specified arch, implant system, temporary restoration and definitive restoration before presenting an offer.
Ask separately about extractions, additional procedures, later visits, repairs and maintenance. Flights are excluded. If more than one trip is required, include the full travel cost when comparing providers.
Plan cleaning, records and future assessment
Ask for instructions specific to the restoration and a demonstration of any recommended care method. Request implant and component details to keep for later dental care.
Does “fixed” mean maintenance-free?
No such promise is made. Ask the dentist to explain checks, care and possible future work.
Is the advertised price for both jaws?
Confirm the unit and covered arch or arches in writing. Do not infer this from a “full-mouth” heading.
Will the first visit complete all treatment?
Ask which stages remain and who provides them. A temporary restoration is not automatically the final agreed treatment.
For a personal full-arch assessment, send your dental history and available imaging through the All-on-4 treatment page. Ask for an arch-specific written proposal.
Agree a contingency if a fixed temporary cannot be fitted
Before extractions or implant placement, ask what the dentist will do if the planned initial fixed bridge cannot be provided. Which alternative restoration could you use, what would it cost, and how would that change eating, appointments or the return journey? This decision belongs in the consultation, before a same-day-restoration expectation becomes part of your travel booking.
Use the FDA implant guide to ask about healing and readiness for the definitive bridge. Ask how readiness is assessed and what happens if healing takes longer than the provisional schedule. Confirm the required assessment before booking the second trip.
Understand which component needs attention if something goes wrong
Ask the dentist to explain the implant body, abutment and fixation screw, and what problems can affect each. Discuss screw loosening, implant failure, infection and nerve injury using the FDA implant guide. Ask what to do about looseness or pain and where prompt assessment is available.
Ask who will assess such a problem at home, what records they will need and how the partner dentist will coordinate any further treatment. Request written terms covering diagnostic work, replacement components, bridge repairs and additional travel. Do not infer that every problem is covered because the original procedure was described as all-inclusive.
What should I budget for the second trip?
Ask for a stage-by-stage quote identifying the definitive restoration, fitting visits and accommodation on the return trip. The catalogue benchmark describes seven nights on trip one; it does not establish the second stay or a confirmed partner offer. Flights remain excluded.
